Community reports on a map, with assigned owners, status tracking and dashboards.
An app for primary healthcare teams to record home visits, fill in forms and place each visit on the map, even without internet. It can be adapted to other field surveys, such as sales or questionnaires.
Under Colombia's new healthcare model, field teams (doctors, nurses, psychologists, therapists and assistants) visit homes to profile people, families and households and identify risk factors.

An installable web app (PWA) with offline mode and sync, GPS capture, a map of homes and forms for each person, family and household. It has dashboards by family and age group, and a routing module that groups homes by distance, fits visits to the workday and suggests the nearest team. Data is stored in PostgreSQL with PostGIS. The system runs on Docker with automatic backups.
The system is in production for healthcare field teams. The same forms-and-map setup works for any field data collection: censuses, sales visits, inspections or surveys.
